Care Act Assessments and Reviews
Jo can support by being at new Care Act assessments or reviews, to ensure all your needs are properly documented and understood. A poor or incomplete assessment can mean missing vital care and support. I can also check the Assessment sent to you.
By law, Care Act assessments should be completed within 28 days of request, and reviews should be carried out annually. We ensure your rights are upheld and your voice is heard.

If requested you are entitled to a Care Act assessment or review, reviews should be carried out annually or when there is a change in need.
The local authority’s policy does not mean it is legal or in Law.
The LA, NHS and housing providers have the responsibility for access to care, treatment, and support. Equal opportunities and Human Rights are law.
If your needs have change and your condition has worsened, you have the right to access continuing health care.
Financial assessments were introduced in 2014.
You have the right to protect your assets, know what you need to do.
